Heart Disease and Stroke Prevention Program

About the Program

The Heart Disease and Stroke Prevention Program was established via a federal capacity-building grant, to reduce the burden of heart disease and stroke among Tennessee residents.  The Program is designed to be implemented in various settings that include communities, hospitals, schools, and work sites.  Currently, the Program is striving to build capacity via:

  • Building infrastructure by strengthening and promoting partnerships with other state and local agencies, schools, universities, healthcare organizations, businesses and non-profit agencies.
  • Collecting and marketing heart disease and stroke data information in Tennessee via development and dissemination of the Burden Document, which describes the health and economic impact of heart disease and stroke
  • Identifying gaps and challenges in clinical systems to address the burden of heart disease and stroke in Tennessee
  • Improving standards of  cardiac and stroke care in the state through evidence-based guidelines
  • Implementing and promoting policy and systems change interventions to address the burden of heart disease and stroke in Tennessee
