The CSMD (Controlled Substance Monitoring Database) was created by an act of the legislature, to be administratively attached to the Tennessee Board of Pharmacy. The state statute that covers this database and its use is TCA 53-10-Part 3, Controlled Substance Monitoring Act of 2002. The Board of Pharmacy and the CSMD Advisory Committee establish, administer, maintain and direct the functioning of the database in accordance with this Part 3.
Pharmacies within the state of Tennessee are required to upload all schedule II-V prescriptions at least twice monthly. Optimum Technology currently has the contract to assist all state of Tennessee pharmacies in the upload and inclusion of their prescriptions into the CSMD database.
Information on uploading prescription data may be obtained from Optimum at 877-683-9771. Our data collection manual may be viewed and printed by clicking here. You may also email Optimum about uploading data at TnRxReport@Otech.com . The data collection website is at www.TnRxReport.com.
Registering to become a user on the CSMD webcenter can be accomplished at https://prescriptionmonitoring.state.tn.us. Click on "Register to become a User" and a single page form will open. The practitioner or pharmacist should complete the page and submit it for consideration. The email address that is given should be a personal and confidential email address, as no one other than the practitioner or pharmacist may have that information. The new user will receive an email with their login credentials for the database.
After March 14, 2012, the CSMD webcenter will be moved to www.TNCSMD.com. For more information about the database, you may contact the CSMD Administrator at CSMD.admin@tn.gov or (615) 253-1305.
