You must be eligible for services according to the guidelines above.

Contact your local county health department or other health center in the county in which you live. You can use this link to locate the address and phone number of your county health department.

The addresses and phone numbers are listed below for the community health centers in Madison County, Shelby County, Davidson County and Hardin County because the health departments are not screening in these counties.

Additional health centers in other areas of the state are also listed. Contact the site closest to where you live and tell them you want to enroll in the Tennessee Breast and Cervical Screening Program and set an appointment time.
