Data includes Profession, Name, Mailing Address and Phone, License and Disciplinary Action indicator.    Data may be further filtered by Profession, Rank and Location.  If the Location is specified as "Tennessee" then a specific County may be selected as well.  The data elements below are optional and may be included by checking the box next to them.  Please be aware that the more data you request, the longer it will take to produce the report.
