EMS Personnel General Information

The goal of the Emergency Medical Services (EMS) licensure section is to promote the training of EMS Personnel based on sound educational principles, current medical knowledge and practice and to issue the individual's license in the most expedient manner possible. The Office of EMS currently licenses Emergency Medical Technicians, Advanced Emergency Medical Technicians and Paramedics and certifies Emergency Medical Dispatchers and Emergency Medical Responders.

Application for Educational Institution Approval

The EMS Division promotes the training of emergency medical services personnel based on sound educational principles, current medical knowledge and practice. The Emergency Medical Services Board approves all educational institutions desiring to train EMT IV, Paramedic and Critical Care Paramedics. Educational Institutions wishing to provide EMS training will make application to the EMS Division. Once application is received and reviewed by the Division a site visit to the institution is scheduled. The EMS Division then makes a recommendation to the EMS Board as to the institutions compliance with EMS Rule: 1200-12-1-.13.

License Information

Initial Licensure

Individuals seeking EMS Professional Certification or License in TN must meet the following Requirements:

Emergency Medical Responder (EMR):

  • Submit a completed application for certification
  • Must be at least seventeen (17) years of age.
  • Must  be able to read, write, and speak the English language.
  • No history within past three (3) years of habitual intoxication or personal misuse of any drugs or intoxicating liquors in such a manner as to adversely affect the ability to practice as an EMR.
  • Must have successfully completed a board approved Emergency Medical Responder training program.
  • Must obtain a passing score on a board approved written examination.
  • Must submit application and certification fees.
  • Must complete are requirements for certification within two years of course completion date.
  • If requirements for certification are not completed within two years of date of initial application the application will be considered abandoned and will be destroyed.

EMERGENCY MEDICAL TECHNICIAN (EMT):

  • Must submit a completed application for licensure.
  • Must be at least eighteen (18) years of age.
  • Must be able to read, write, and speak the English language.
  • Must possess an academic high school diploma or a general equivalency diploma (G.E.D).
  • Must have no history within the past three years of habitual intoxication or personal misuse of any drugs or the use of intoxicating liquors, narcotics, controlled substances, or other drugs or stimulants in such manner as to adversely affect the person’s ability to practice as an emergency medical technician.
  • Must present evidence to the Office of Emergency Medical Services of a medical examination certifying physical health sufficient to conduct activities associated with patient care, including, but not limited to, visual acuity, speech and hearing, use of all extremities, absence of musculoskeletal deformities, absence of communicable diseases, and suitable emotional fitness to provide for the care and lifting of the ill or injured. This information shall be provided on a form approved by the Board and shall be consistent with the provisions of the Americans with Disabilities Act and the requirements of National Registry of Emergency Medical Technicians.
  • Must successfully complete an EMS Board approved Emergency Medical Technician course.
  • Must achieve a passing score on a EMS Board approved written examination.
  • Must successfully complete an EMS Board approved practical examination.
  • Must complete a criminal background check from the State approved vendor.
  • Must pay all required application and license fees.
  • Must disclose circumstances surrounding any conviction of any criminal law violation other than minor traffic violations, denial of professional licensure/certification, discipline, loss or restriction of licensure or certification by any other state or discipline.
  • Must complete entire license process within two years of course completion.
  • If requirements for certification are not completed within two years of date of initial application the application will be considered abandoned and will be destroyed.

ADVANCED EMERGENCY MEDICAL TECHNICIAN (AEMT):

  • Must submit a completed application for licensure.
  • Must be at least eighteen (18) years of age.
  • Must be able to read, write, and speak the English language.
  • Must possess an academic high school diploma or a general equivalency diploma (G.E.D).
  • Must have no history within the past three years of habitual intoxication or personal misuse of any drugs or the use of intoxicating liquors, narcotics, controlled substances, or other drugs or stimulants in such manner as to adversely affect the person’s ability to practice as an emergency medical technician.
  • Must present evidence to the Office of Emergency Medical Services of a medical examination certifying physical health sufficient to conduct activities associated with patient care, including, but not limited to, visual acuity, speech and hearing, use of all extremities, absence of musculoskeletal deformities, absence of communicable diseases, and suitable emotional fitness to provide for the care and lifting of the ill or injured.   • Must successfully complete an EMS Board approved practical examination.
  • Must complete a criminal background check from the State approved vendor.
  • Must pay all required application and license fees.
  • Must disclose circumstances surrounding any conviction of any criminal law violation other than minor traffic violations, denial of professional licensure/certification, discipline, loss or restriction of licensure or certification by any other state or discipline.
  • Must complete entire license process within two years of course completion.

PARAMEDIC:

  • Must submit a completed application for licensure.
  • Must be at least eighteen (18) years of age.
  • Must be able to read, write, and speak the English language.
  • Must possess an academic high school diploma or a general equivalency diploma (G.E.D).
  • Must hold a current Tennessee license as an AEMT.
  • Must have no history within the past three years of habitual intoxication or personal misuse of any drugs or the use of intoxicating liquors, narcotics, controlled substances, or other drugs or stimulants in such manner as to adversely affect the person’s ability to practice as an emergency medical technician.
  • Must present evidence to the Office of Emergency Medical Services of a medical examination certifying physical health sufficient to conduct activities associated with patient care, including, but not limited to, visual acuity, speech and hearing, use of all extremities, absence of musculoskeletal deformities, absence of communicable diseases, and suitable emotional fitness to provide for the care and lifting of the ill or injured. This information shall be provided on a form approved by the Board and shall be consistent with the provisions of the Americans with Disabilities Act and the requirements of National Registry of Emergency Medical Technicians.

  • Must complete entire license process within two years of course completion.
  • If requirements for certification are not completed within two years of date of initial application the application will be considered abandoned and will be destroyed.

Paramedic Critical Care:

  • Must submit a completed application for Licensure.
  • Must meet all the Emergency Medical Technician Paramedic requirements in rule 1200-12-1- .04(2) and must be currently licensed as a paramedic in good standing in Tennessee.
  • Must submit completed application and fees.
  • Successful completion of a Board approved Critical Care Paramedic Program.
  • Successful completion with a passing score as established by the Board on an approved endorsement examination.
    • After the third attempt to complete the exam an individual must show remediation from the critical care program before they may take the examination again.
    • After six unsuccessful attempts in less than two years, the individual must repeat the entire training before attempting examination.
  • Endorsement must be obtained within two years of completion of an approved course.
  • Individuals completing a Critical Care Paramedic Course prior to July 2007 may make application for endorsement.
    • Individuals must show documentation of completion of a Critical Care Paramedic Course prior to July 2007.
    • Successful completion with a passing score as established by the Board on an approved endorsement examination.
  • Individuals will have two (2) years to complete the examination requirements before being required to repeat the course.

Renewal Requirements

EMS Professional renewal date is based on the individual’s birth month and year. EMS Professional license will expire on the last day of the individual’s birth month in either an odd or even year depending on whether the individual was born in an odd or even year. Example: Birthday 12-05-1972, expiration date, 12-31-2010.


REQUIREMENTS


Emergency Medical Responder:

  • Payment of renewal fees
  • Current Cardiopulmonary Resuscitation Healthcare Provider or equivalent
  • Successful completion of Board approved refresher training course
  • OR
  • Successful completion for an EMS Board approved written examination
  • OR
  • Completion of ten (10) continuing education hours approved by the EMS Board. Two (2) hours must be in pediatric related topics
  • And
  • Documentation of skills competency
  • OR
  • 1 college credit hour in EMR related studies
  • And
  • Documentation of skills competency

Emergency Medical Technician

  • Payment of renewal fees
  • Current Cardiopulmonary Resuscitation Healthcare Provider or equivalent
  • Successful completion for an EMS Board approved written examination
  • OR
  • Completion of twenty (20) continuing education hours approved by the EMS Board. Five (5) of the twenty (20) hours must be in pediatric related topics
  • OR
  • Completion of two (2) college credit hours in EMT related topics

Emergency Medical Technician IV :

May renew until December 31, 2016 after which EMTIV must have completed Transition to AEMT or downgraded license to EMT

  • Payment of renewal fees
  • Current Cardiopulmonary Resuscitation Healthcare Provider or equivalent
  • Successful completion for an EMS Board approved written examination
  • OR
  • Completion of twenty (20) continuing education hours approved by the EMS Board. Five (5) of the twenty (20) hours must be in pediatric related topics
  • OR
  • Completion of two (2) college credit hours in EMT related topics

Advanced Emergency Medical Technician:

  • Payment of renewal fees
  • Current Cardiopulmonary Resuscitation Healthcare Provider or equivalent
  • Successful completion for an EMS Board approved written examination
  • OR
  • Completion of twenty-five (25) continuing education hours approved by the EMS Board. Eight (8) of the twenty –five (25) hours must be in pediatric related topics
  • OR
  • Completion of two (2) college credit hours in AEMT related topics

Paramedic:

  • Payment of renewal fees
  • Current Cardiopulmonary Resuscitation Healthcare Provider or equivalent
  • Successful completion for an EMS Board approved written examination
  • OR
  • Completion of thirty-two (32) continuing education hours approved by the EMS Board. Eight (8) of the thirty-two (32) hours must be in pediatric related topics
  • OR
  • Completion of three (3) college credit hours in Paramedic related topics

Paramedic Critical Care:

  • Payment of renewal fees
  • Same renewal requirements as Paramedic AND
  • Completion of 10 hours of additional continuing education contact hours in critical care topics. These hours may include, but are not limited to, continuing education, in-service and attendance and participation in other approved critical care programs.

IF YOU ELECTED TO RECEIVE NOTICE BY EMAIL AND ARE NOW WISHING TO SEND YOU A RENEWAL BY MAIL YOU MUST DOWNLOAD THE EMS PROFESSIONAL FEE FORM (PH 2397) UNDER THE TAB FORMS FOR EMS LICENSURE. THIS FORM MUST BE COMPLETED, SIGNED AND MAILED WITH YOUR FEES.

Retirement of License

The Emergency Medical Services Rules and Regulations now allow for individuals who so desire to retire their Professional EMS License. Any currently licensed EMS Professional who wishes to permanently retire his or her license must submit the properly completed permanent retirement affidavit form to the Office of Emergency Medical Service. Retired license may be reinstated in accordance with the Emergency Medical Services Rules 1200-12-1-.04

Reinstatement

EMS Professionals whose license have expired and are within 60 days of their expiration date and have met renewal requirements prior to expiration date must complete the following:

  • Submit completed EMS Professional Fee Form (PH 2397), renewal fee and late fee
  • Submit proof of required continuing education completed prior to expiration date
  • OR
  • Obtain a passing score on a board approved written examination for level reinstating prior to expiration date

EMS Professionals which have a license expired greater than 60 days of expiration date but less than one year or who did not complete continuing education or pass exam prior to expiration date must complete the following:

  • Submit completed EMS Professional Fee Form (PH 2397), renewal fee and reinstatement fee
  • Obtain a passing score on a board approved written examination for level reinstating.

EMS Professionals which have a license expired greater than one year but less than two years must:

  • Submit completed EMS Professional Fee Form (PH 2397), renewal fee and reinstatement fee
  • Successfully complete EMS Board approved refresher course for level reinstating
  • Obtain a passing score on an EMS Board approved written and practical examination for level reinstating.

EMR, EMT or AEMT License expired greater than two years must complete the board approved training course for appropriate level of licensure in the entirety and comply with initial license requirements in effect under rule 1200-12-01-.04 (3)(a).

Paramedic license expired greater than two years

  • Submit proof of completion of a state approved Paramedic Training Program taken after January 1, 1977;
  • Show proof of previous Tennessee licensure as a Paramedic;
  • Successfully complete Board approved written and practical examinations;
  • Submit a current Advanced Cardiac Life Support (ACLS) provider or instructor certification from the American Heart Association;
  • Submit a current Pre-hospital Trauma Life Support (PHTLS) or International Trauma Life Support (ITLS) certification as a provider or instructor;
  • Submit a current Pediatric Advanced Life Support (PALS) certification as a provider or instructor;
  • Complete a state approved Paramedic Refresher Training Course or complete forty-eight (48) hours of Advanced Life Support training that overviews the topical content of the state approved Paramedic Refresher Training Course
  • Submit completed EMS Professional Fee Form (PH 2397) ,renewal fee and reinstatement fee
  • Submit two recent (within the preceding 12 months) original letters of good moral character from medical professionals attesting to applicant’s personal character.
  • Must complete a criminal background check from the State approved vendor.
    • The background check shall be valid for one (1) year from the date it is obtained
  • Submit a current copy of Cardiopulmonary Resuscitation Healthcare Provider card or equivalent;
  • Submit a medical examination certifying physical health sufficient to conduct activities associated with the profession on Medical Form (PH 0130) approved by the Board and consistent with the provisions of the Americans with Disabilities Act.
